What to expect at initial visit
Patient or parent/guardian will need:
- Valid ID
- Copy of insurance card
All co-payments are due at the time of service.
For first-time patients, please arrive 20 minutes prior to the initial appointment for completion of new patient paperwork. If applicable, a legal guardian must present guardianship paperwork.
Medication Management
Our goal is to optimize drug therapy and improve the therapeutic outcomes for all of our patients. Including
- patient assessments
- comprehensive medication review
- formulating an effective medication treatment plan
- monitoring efficacy and safety of medication therapy
As part of our care plan, we will develop a personal medication record. This will include: all prescription and nonprescription products and will be updated as needed. After we document your medication records, we will develop a full plan of self-management actions necessary to achieve your goals.
