What to expect at initial visit
All co-payments are due at the time of service.
For all new patients, please make sure your electronic new patient forms are completed in your patient portal at least one (1) week prior to your appointment so that your provider will have time to review all of the forms prior to your appointment.
Medication Management
Our goal is to optimize drug therapy and improve the therapeutic outcomes for all of our patients. Including
- patient assessments
- comprehensive medication review
- formulating an effective medication treatment plan
- monitoring efficacy and safety of medication therapy
As part of our care plan, we will develop a personal medication record. This will include: all prescription and nonprescription products and will be updated as needed. After we document your medication records, we will develop a full plan of self-management actions necessary to achieve your goals.
